An Alberta Health Services call center that serves as a single point of contact for care providers which facilitates the return of patients to a Health Care facility closest to their home address that will best meet the patient’s health care needs (repatriation).

Our Purpose

To provide Alberta Health Care professionals with a single point of contact in order to:

• Seamlessly enable physicians to access a higher level of care for patients
• Coordinate patient flow through the repatriation process

How to Start the Process

NOTE: Prior to proceeding with the repatriation three criteria must be met.

The patient must:

• have a valid Alberta Health Care number.
• have insurance coverage to pay for transportation costs (for repatriation internationally or within Canada – does not apply to repatriations within Alberta). If the patient does not have appropriate insurance, transportation arrangements must be discussed with the patient, family or other applicable agency PRIOR to patient transfer.
• be FIT TO FLY based on the sending physician’s medical assessment.

To proceed:

1. Download and print the RAAPID Patient Repatriation Request.
   Note: If you are unable to access the form, call RAAPID North or RAAPID South and forms can be faxed to you.
2. Sending hospital staff must complete the Repatriation form with the following information:
   • The most responsible practitioner name and contact information
   • Relevant information regarding the patient’s care needs, procedures, past medical history, and present medical status;
   • Complete patient demographics including date of birth, valid provincial health care number, and patient’s current address
3. Fax the completed forms back to RAAPID or scan and e-mail to raapid@ahs.ca, and wait for a RAAPID nurse to contact the sending site regarding next steps.
